*Employees and their dependents enrolled in the Select Plan and who use SJGH facilities and physicians exclusively will receive a 50% reduction in medical deductibles. During open enrollment, you must complete and submit to County Human Resources the reduced deductible form available at Human Resources. You can also obtain the form from the County's open enrollment web site at http://www.sjgov.org/hr/, click on Open Enrollment 2008-09 and choose HCS Exclusive Use Form.
